We, MARIA C. CRUZ, JOSE C. CRUZ, IMELDA C. CRUZ and SIMON C. CRUZ all of legal age, Filipino citizens and residents of Barangay Poblacion, Makati City Philippines, after having been duly sworn to in accordance with the law, depose, and state that:
I, MARIA C. CRUZ is the surviving spouse of the deceased PEDRO M. CRUZ who died on 01 January 2015 at Makati City, Philippines.
That we, JOSE C. CRUZ, IMELDA C. CRUZ and SIMON C. CRUZ are the only children of PEDRO M. CRUZ and MARIA C. CRUZ.
That we are executing this affidavit to attest to the truth and veracity of the foregoing facts and for all legal intents and purpose that it may serve.
