Affidavit of Request and Consent for Waiver of Exclusion Ground (WEG)

Non-Filipino minors below fifteen (15) years of age, who are traveling unaccompanied and are not joining a parent in the Philippines, are required to apply for a WAIVER of EXCLUSION GROUND (WEG) at the Philippine Embassy in order to be admitted entry in the Philippines regardless of whether or not the said minor requires a visa. 

1. Submission of Affidavit of Request and Consent for WEG by either parent or legally appointed guardian of the child, naming the person who will be accompanying the child to the Philippines and with whom the child will reside while in the Philippines, and 

(a) Clear photocopy of the data page of the child’s passport. 

(b) Clear photocopy of the data page of the passport of the accompanying adult or guardian. 

2. Payment of notarization fee at the Philippine Embassy for the Affidavit of Request and Consent for WEG 

3. Payment of the WEG processing fees at the Bureau of Immigration-NAIA upon arrival of the minor child/children.

FOR WAIVER OF EXCLUSION GROUND (WEG)

    I/WE, _______________________, of legal age, Filipino(s), single/married/widow/widower and presently residing at _______________________ after having been duly sworn to in accordance with law, depose and state:

1. That I/we am/are we are the biological parents and /or person exercising parental authority of minor _______________ who was born on ____________ in _____________, now aged _____ years old.

2.  That said child will travel to the Philippines for the following reason/s: ______________________________________;

3. That said child will be leaving on _____________________ and will reside at ______________________ (address in the Philippines) for a period of _________________ (number of days) and will return with ______________________ (name of companion);

4. That I/we cannot personally travel with my/our child due to: _________________________________ (specify reasons);

5. That I am/we are giving my/our full consent for the minor child to travel abroad to be accompanied by _________________________;

6. That as his parent/legal guardian, I hereby accept and assume any and all responsibility for his/her welfare, including all financial expenses related to his/her travel and stay in the Philippines; 

7. That this affidavit is being executed for presentation to the Philippine Immigration authorities at the port of entry that a waiver may be granted in favor of my child, allowing him/her entry to the Philippines if admissible, under Section 29 (a) (12) of the Philippine Immigration Act (Waiver of Exclusion Ground).
